Key Responsibilities – Nurse Practitioner, Post-Acute Care
- Deliver exceptional medical care to patients in post-acute and long-term care environments
- Collaborate proactively with multidisciplinary teams to help achieve optimal patient outcomes
- Utilize Electronic Medical Records (EMR) accurately for timely patient management and documentation
- Build professional and compassionate relationships with patients, families, and healthcare teammates
Essential Qualifications and Skills – Nurse Practitioner, Charlottesville VA
- Active NP degree from an accredited nursing program
- Valid and unrestricted Virginia Nurse Practitioner license and DEA registration
- Confidence and experience using EMR systems and clinical documentation
