Role Overview: PMHNP — Part Time, Windsor, VA
Legacy Care is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team on a part‑time basis in Windsor, VA. The successful clinician will provide psychiatric assessment, diagnosis and management for residents in post‑acute and long‑term care settings. This part‑time role requires strong clinical judgement, excellent communication skills, and a commitment to trauma‑informed, person‑centered, evidence‑based psychiatric care. Core responsibilities include mental health evaluations, medication management, interdisciplinary collaboration and participation in quality and safety initiatives.

Key Responsibilities — PMHNP, Post‑Acute Care
Conduct psychiatric assessments and develop individualized mental health care plans for residents in post‑acute and long‑term care settings
Diagnose and manage psychiatric disorders, including mood, anxiety, psychotic and neurocognitive conditions, consistent with current evidence‑based practice
Provide medication management for psychotropic medications, including monitoring for efficacy, side effects and interactions, coordinate medication reconciliation
Deliver brief psychotherapeutic interventions and supportive counselling appropriate to the setting and scope of practice
Collaborate with nursing staff, physicians, therapists, social workers and facility leadership to deliver holistic, person‑centered care
Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences
Respond promptly to changes in psychiatric or behavioral status, initiate appropriate interventions and coordinate transfers of care when necessary
Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
Support medication safety and stewardship initiatives, including minimization of inappropriate antipsychotic use and attention to polypharmacy
Provide clinical consultation and education to facility staff and colleagues on behavioral management, de‑escalation techniques and mental health best practices
Maintain professional standards of practice, including infection control, documentation and regulatory compliance
Essential Qualifications and Skills — PMHNP, Windsor VA
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) with PMHNP certification from an accredited institution
Current and unrestricted Virginia Nurse Practitioner license and APRN certification
Clinical experience in psychiatric care, geriatrics, long‑term care or post‑acute settings preferred
Proficiency with Electronic Medical Record systems and clinical documentation
Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams
Commitment to trauma‑informed, person‑centered care, cultural sensitivity and ethical practice
Current Basic Life Support (BLS) certification required; additional certifications relevant to behavioral emergencies preferred
