Role Overview: PMHNP — Part Time — Hampton, VA
We are seeking a dedicated and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our Hampton, VA team on a part‑time basis. The successful clinician will deliver mental health assessment and treatment services within post‑acute and long‑term care settings, working closely with facility staff to support resident‑centered behavioral health, symptom management and overall wellbeing.

Key Responsibilities
Provide high‑quality psychiatric assessment, diagnosis and evidence‑based treatment for residents in post‑acute and long‑term care settings
Conduct timely on‑site and telehealth mental health evaluations, risk assessments and crisis management as needed
Develop and implement individualized psychiatric care plans, setting measurable goals in collaboration with residents, families and facility staff
Manage psychopharmacologic therapies, including initiation, monitoring, reconciliation and deprescribing where appropriate
Provide psychotherapy and supportive counselling appropriate to the population and scope of practice
Coordinate care with primary care clinicians, nursing staff and behavioral health specialists, and facilitate referrals to community resources and psychiatry when required
Document mental health encounters accurately and promptly in the Electronic Medical Record (EMR)
Participate in care‑planning meetings, multidisciplinary rounds and family conferences to ensure coordinated, person‑centered care
Support facility quality and safety initiatives related to behavioral health, including reduction of psychotropic polypharmacy, fall prevention and minimization of restrictive interventions
Qualifications and Required Skills
Master’s degree or post‑graduate PMHNP qualification from an accredited nursing program
Active and unrestricted Nurse Practitioner license in Virginia and current DEA registration or waiver as required
Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP‑BC or equivalent) preferred
Experience in geriatric psychiatry, post‑acute care, long‑term care or related behavioral health settings preferred
Proficiency with Electronic Medical Record systems and accurate clinical documentation
Excellent clinical judgement, strong communication skills and the ability to work collaboratively within interdisciplinary teams
Current Basic Life Support (BLS) certification required; additional certifications desirable depending on facility needs
Compassionate, culturally sensitive and professional approach to resident‑centered mental health care
