Role Overview: Nurse Practitioner - PRN - Charlottesville, VA
We are seeking a skilled and compassionate Nurse Practitioner to join our Charlottesville team on a PRN basis. This role offers the opportunity to provide high-quality, person-centered primary and post-acute care within skilled nursing, long-term care and assisted living settings. The successful clinician will work collaboratively with interdisciplinary teams, support patients and families, and promote continuity of care across the healthcare continuum.

Key Responsibilities
Provide comprehensive medical evaluations, assessments and follow-up care for patients in post-acute, long-term care and assisted living settings
Diagnose and manage acute and chronic medical conditions, including complex and age-related health concerns
Develop, implement and revise individualized treatment plans in collaboration with patients, families, facility staff and interdisciplinary teams
Perform physical examinations, review medical histories and interpret relevant laboratory and diagnostic results
Prescribe and manage medications safely and effectively, including monitoring treatment response, side effects and potential interactions
Identify changes in patients’ condition and provide timely assessment, intervention and escalation of care when clinically indicated
Provide education and support to patients, families and facility staff regarding diagnoses, treatment plans, medications and preventive care
Participate in care planning meetings, interdisciplinary reviews and family consultations as required
Coordinate care with primary care clinicians, specialists, hospitals, rehabilitation providers and community resources
Support transitions of care and promote continuity, safety and quality across healthcare settings
Maintain accurate, timely and clinically appropriate documentation in the Electronic Medical Record (EMR)
Contribute to quality improvement initiatives and promote trauma-informed, culturally sensitive and person-centered care
Qualifications and Required Skills
Master’s or doctoral degree in Nursing from an accredited institution
Current, unrestricted Virginia Nurse Practitioner license and eligibility to practice as an NP in Virginia
Current national Nurse Practitioner board certification through an approved certifying organization
Active DEA registration and prescriptive authority, as required
Experience in primary care, post-acute, skilled nursing, long-term care, geriatric or related settings preferred
Strong clinical assessment, diagnostic reasoning and treatment management skills
Proficiency with Electronic Medical Record systems and timely, accurate documentation practices
Excellent communication, collaboration and problem-solving abilities
Compassionate, culturally sensitive and person-centered approach to patient care
Ability to work independently while collaborating effectively with facility staff and multidisciplinary care teams
Ability to provide reliable PRN coverage based on patient and facility needs
