Role Overview: Physician — PRN, Warrenton, VA
We are seeking a skilled and compassionate Physician to join our team in Warrenton, VA on a PRN (as‑needed) basis. The successful clinician will provide comprehensive primary and acute care services for residents in post‑acute and long‑term care settings, with an emphasis on stabilising acute issues, managing complex chronic conditions and delivering person‑centred care. This role requires excellent clinical judgement, effective communication skills and the ability to collaborate closely with interdisciplinary facility teams while working flexible shifts.

Key Responsibilities — Physician, Post‑Acute Care
Provide timely on‑site assessment, diagnosis and treatment of acute changes in clinical status for residents in post‑acute and long‑term care settings
Manage chronic medical conditions with an emphasis on preserving function, minimising hospitalisations and optimising quality of life
Perform comprehensive assessments, develop individualised care plans and set measurable goals in collaboration with residents, families and facility staff
Order and interpret diagnostic tests, initiate appropriate treatments and coordinate specialist referrals as needed
Provide medication management, including reconciliation, monitoring for efficacy and adverse effects, and deprescribing when appropriate
Coordinate transitions of care, including admissions, transfers and discharges, and communicate effectively with hospitals, families and community providers
Participate in care‑planning meetings, interdisciplinary rounds and family conferences to ensure holistic, person‑centred care
Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
Support facility quality and safety initiatives, including infection prevention, falls reduction and medication safety
Provide clinical leadership and education to nursing staff and facility teams on acute management, chronic disease care, palliative approaches and best practice
Maintain professional standards of practice and ensure compliance with regulatory and facility requirements
Essential Qualifications and Skills
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
Current and unrestricted Virginia medical license required
Board certification in a relevant specialty preferred; experience in geriatrics, internal medicine or family medicine is highly desirable
Current DEA registration and any other required state credentials
Proficiency with Electronic Medical Record systems and accurate clinical documentation
Strong clinical judgement, interpersonal and communication skills; ability to work collaboratively within multidisciplinary teams
Commitment to trauma‑informed, person‑centred care, cultural sensitivity and ethical practice
Current Basic Life Support (BLS) certification required; advanced certifications (ACLS, PALS) desirable depending on facility needs
