Role Overview: Nurse Practitioner - Part Time - Warrenton, VA
We are seeking a dedicated and compassionate Nurse Practitioner to join our Warrenton, VA team on a part‑time basis. This role provides an opportunity to make a meaningful impact on post‑acute patient care within skilled nursing and long‑term care facilities, working alongside facility staff to support resident‑centered outcomes.

Key Responsibilities
Provide high‑quality primary and acute medical care to residents in post‑acute and long‑term care settings
Perform timely on‑site assessments, diagnosis and management of acute clinical changes
Manage chronic conditions with a focus on preserving function, optimizing quality of life and reducing avoidable hospital admissions
Develop individualized care plans and set measurable goals in collaboration with residents, families and facility staff
Order and interpret diagnostic tests, initiate treatments and coordinate specialist referrals as required
Provide safe and effective medication management, including reconciliation and deprescribing when appropriate
Document patient encounters accurately and promptly in the Electronic Medical Record (EMR)
Collaborate with multidisciplinary teams through rounds, care‑planning meetings and family conferences to ensure coordinated, person‑centred care
Support facility quality and safety initiatives such as infection prevention, falls reduction and medication safety
Qualifications and Required Skills
Master’s degree or post‑graduate NP qualification from an accredited nursing programme
Active and unrestricted Nurse Practitioner license in Virginia and current DEA registration as required
Experience in post‑acute, long‑term care, gerontology or related settings preferred
Proficiency with Electronic Medical Record systems and accurate clinical documentation
Excellent clinical judgement, strong communication skills and the ability to work collaboratively within interdisciplinary teams
Current Basic Life Support (BLS) certification required; additional certifications (e.g. ACLS) desirable depending on facility needs
